Parental consent forms

What is a Parental Consent Form? 

Users who are under the age of 13 and on a free bulb account will need to submit a Parental Consent Form to gain access to their account.

1. Create a bulb account at https://www.bulbapp.com/b/sign-up
2. Upon creating an account, you will be prompted to complete and submit a Parental Consent Form.
3. When completing the form, please be sure to provide the username and email address that your child used when signing-up for his or her bulb account.
4. Email your Parental Consent Form to help@bulbapp.com; the bulb Care Team will unlock your child's bulb account.
5. Get Creating!
